Before leaving the APM each participant should drop off the envelope containing completed CEU session evaluation forms to APM registration to ensure the most efficient processing. Participants in sessions scheduled after the close of registration should mail their envelopes with completed CEU session evaluation forms to: Council on Social Work Education, 1701 Duke Street, Suite 200, Alexandria, VA 22314.
CSWE staff will review the contents of each envelope for proper completion of forms, confirm receipt of the processing fee, and tally the credits earned. CEU certificates will be transmitted electronically within 30 days of the APM.
NOTE: Without the CEU evaluation forms from each session attended as proof of attendance, your state licensing board may not acknowledge your credits earned.
