Using the CEU Passport, participants are asked to complete a brief written evaluation of the session objectives and the presenters of each CEU session attended. Each CEU Passport will contain one 8.5 x 11” evaluation form for every CEU eligible session and a return envelope. CEU participants must:
- COMPLETE the appropriate evaluation form,
- PLACE all completed evaluation forms into the envelope, and
- DROP OFF the envelope at APM registration before leaving the APM.
Participants in sessions scheduled after the close of registration should mail their envelopes with completed CEU session evaluation forms to: Council on Social Work Education, 1701 Duke Street, Suite 200, Alexandria, VA 22314.
