Reaffirmation—2015 Educational Policy and Accreditation Standards (EPAS)

Preparation

Attend a Reaffirmation Workshop
Sign up for a Reaffirmation Workshop hosted by CSWE staff members. This hybrid training model includes online workshop modules available on-demand and an all-day, face-to-face workshop. More information regarding registration and workshop content can be found here.

Eligibility Materials

Authorization of Program Review 
Form used by the president of the social work program's institution to authorize a review of the social work degree program. This should accompany the Reaffirmation Eligibility Application. 

Reaffirmation Eligibility Application (Baccalaureate)
Baccalaureate social work program eligibility application for reaffirmation of accredited status. The program must demonstrate that its host institution can and will support and sustain a baccalaureate social work program. 

Reaffirmation Eligibility Application (Master’s)
Master’s social work program eligibility application for reaffirmation of accredited status. The program must demonstrate that its host institution can and will support and sustain a master’s social work program.

Site visit planning form for programs reviewed for reaffirmation of accreditation at the February meeting of the CSWE Commission on Accreditation. Programs are to download and complete the form and then e-mail it to the CSWE site visit coordinator.

Self-Study Forms

Accreditation Review Brief (2015 EPAS)
The program is to download and complete its portion of the Accreditation Review Brief and send it to the accreditation specialist along with the program’s self-study. The Accreditation Review Brief is used by the CSWE Commission on Accreditation reader to report his or her evaluation of the program reviewed for reaffirmation of accreditation.

Assessment Reporting Form—Baccalaureate (2015 EPAS)
Programs use this form to report student learning outcomes on their websites. Programs are required to post this form on their websites, updating the form at least every 2 years, and provide the URL in their self-study documents.

Assessment Reporting Form—Master's (2015 EPAS)
Programs use this form to report student learning outcomes on their websites. Programs are required to post this form on their websites, updating the form at least every 2 years, and provide the URL in their self-study documents.

Budget Form (2015 EPAS) 
This form is used to evaluate a program's compliance with Accreditation Standard 3.4.1 during their review for reaffirmation of accreditation. 

Faculty Data Form (2015 EPAS)
This form provides supporting documentation and formatting guidance for CVs of all faculty members teaching in the current academic year to be included in the self-study for AS 3.2.1, AS 3.2.2, AS B3.2.4, and AS M3.2.4.

Faculty Summary Form (2015 EPAS)
This form provides supporting documentation for all faculty members teaching in the current academic year to be included in the self-study for AS 3.2.1, AS 3.2.2, AS B3.2.4, and AS M3.2.4.

Librarian’s Report Form (2015 EPAS)
This report is used to evaluate the program's compliance with AS 3.4.4. 

Site Visits

Purpose and Overview of the Site Visit
Each program going through reaffirmation has a site visit. The site visit occurs after the CSWE Commission on Accreditation reviews the program’s self-study and issues a letter of instruction to the site visitor. All site visitors are expected to discuss general questions related to three standards with programs: AS 1.0 (program mission and goals), AS 3.0 (diversity), and AS 4.0 (assessment). In addition, standards about which the Commission on Accreditation has raised specific questions will be addressed during the site visit.
